Wabash College Student Health Center

Release of Medical Records Form

Dear Wabash Students:

We want to ensure you have continuity of care as you transition from campus to home. If you would like us to send your Student Health Center records to a health care provider at home, so they can pick up where we left off with your care, please fill out this Release of Information form and email it to Nurse Amidon,

We now have a HIPAA-compliant health care Zoom account as well, so we can offer Zoom visits for those of you who are not on campus, until you can make other arrangements for your care.

Nurse Amidon is reaching out to students who have regularly-scheduled appointments to offer this, but feel free to email her to request a virtual appointment.

Take good care of yourselves and each other during these challenging times.

--Nurse Amidon, Dr. Douglas, & Dr. Roberts
