Prior to taking classes from Wabash College, all students must provide the Student Health Center with current address, emergency contact, insurance, medical alert, and health history information. Everything must be filled out in its entirety and a copy of your insurance card (front and back). To expedite this process Wabash College uses an online data entry system.
